The use of PPE in the treatment of men with erectile dysfunction is still controversial and there are no controlled data from human clinical trials to support its use. The main aim of this study was to assess the effects of PPE on blood clots in men with erectile dysfunction who are taking sildenafil or tadalafil. The study will include a total of 30 men with erectile dysfunction who had a baseline body mass index (BMI) of 18.5 or over. All men were instructed to take sildenafil (Viagra, Pfizer) or tadalafil (Cialis), the main active ingredient of sildenafil and tadalafil, for one week. The men were randomly assigned to take sildenafil (5 mg), tadalafil (5 mg), or the main active ingredient of sildenafil and tadalafil. They were also told that their blood clots were assessed with ultrasound before and one month after treatment, and their height and weight were recorded. The men were followed up for six months. In addition, the men were asked to record their sexual activity habits for at least 6 months. Results of the study were discussed in a meeting of the Sexual Medicine Association of the International Association for the Study of Men's Health (SISH) on November 5, 2019.

The results of this study are in agreement with the results of the literature on the effect of PPE on blood clots.
In a study in rats that used a combination of sildenafil and tadalafil, sildenafil, the highest concentration of sildenafil was found in the group of rats treated with 25 mg/kg/day sildenafil and tadalafil. A similar high concentration of sildenafil was found in the group treated with 25 mg/kg/day sildenafil and tadalafil. In the study, there were no significant differences in the plasma concentration of sildenafil or tadalafil after treatment with sildenafil or tadalafil at any concentration. However, a higher concentration of sildenafil was observed in the group treated with 25 mg/kg/day sildenafil and tadalafil, but not in the control group. In contrast, in the group treated with 25 mg/kg/day sildenafil and tadalafil, the plasma concentration of sildenafil was reduced by 30%. In the group treated with 25 mg/kg/day sildenafil and tadalafil, the plasma concentration of sildenafil increased by 37.5% and by 39.5% compared to the control group, respectively. The increase in the plasma concentration of sildenafil was not significantly different between the treated group and the control group, and the plasma concentration of tadalafil was not different.
There is no evidence that sildenafil and tadalafil affect the blood clots in men with erectile dysfunction. Therefore, the effects of PPE on blood clots should be considered when considering the use of sildenafil, tadalafil, or any other ED treatment.
